Membership: Auxiliary Forms
The most commonly used Auxiliary forms are listed below for your reference. Please click on the form name if you'd like to view or print the form.
- Contributions/Donations Log
This form is used to record monthly donations.
- Donation Form
This form is filled out for each memorial/honorarium received by an Auxiliary. This form is mailed to the Memorials & Honorariums director.
- Memorial Permanent Record
This form is used to record each memorial and honorarium received by the Womens General Auxiliary. These forms are permanently filed in the Memorial Book and are on display in the Auxiliary sunroom.
- Packing List
This form is a Packing List to be enclosed in each box of donations sent to the Twin Cities Womens General Auxiliary.
- Estate DeclarationThis form is used when receiving funds from an estate.
